Health Test Pre-Test Instructions

Thank you for booking your health test at Weta digital. We cannot emphasise enough how important is it that you read and complete ALL pre test instructions below. If you do not complete ALL pre-test requirements you will NOT be able to have your health test and you will NOT be offered a refund. 

Step 1:

It is a requirement that all crew working from home complete a Weta Health Declaration form before coming in for their health test. See information below, then return to these pre-test instructions for next steps.

  • Download the editable health declaration here (open in preview, not adobe acrobat)
  • Complete and send to askhr@wetafx.co.nz AT LEAST 48 hours before your health test. (NOTE: This form expires after two weeks - ideally fill in and submit within a week of your appt)
  • In the subject line please write 'Health Checks 2020' and the date of your appointment (e.g Oct 25th)
  • HR will clear you, so expect an email from them confirming this (Treat this email as final confirmation of your appointment)

The health of our clients and staff members is our number 1 priority. If you have had a positive test for COVID-19 within the past month please do NOT book in for any of our face-to-face services. If you have current symptoms of COVID-19 please do NOT turn up to your health test. If you have any questions about this please email our director peter@healthandfitnesstesting.nz

Note: For your safety, Health and Fitness Testing NZ will be communicating with a Weta Digital representative before your health test. We will share your name with Weta Digital (and vice versa) to ensure that you have completed the Weta Health Declaration form prior to coming in. No form completed = no health test. 

Step 2:

Prior to your 1st Fit3D Body Scan/Advanced Test Pack, it is essential that you create a FIT3D account. Please create a Fit3D account here, take note of your password, and return to read the remainder of these pre-test instructions once you have signed up. You will receive a welcome email from Fit3d if you have done this correctly (please check your junk/trash folders). 

If you have had a Fit3d Body Scan before, you can check your login details by visiting www.fit3d.com and clicking login.

Step 3:

Please take note of the location of your Health test. If you have not been to this location before, we strongly recommend you arrive 10 min early to ensure we are able to complete all of your tests.

Privacy 

Our privacy policy can be viewed here. We do use some 3rd party software to store your results. The Fit3D platform, which allows you to view (and store) your body scan results, is a third party and has its own Term & Conditions separate from Health and Fitness NZ. You can view Fit3d’s T’s and C’s here and privacy policy here.

Health and Fitness Testing Staff will have access to all your test results. Health and Fitness Testing will not share your specific results (e.g. you cholesterol, blood pressure, HbA1c/diabetes results) with Weta Digital, your doctor, or anyone without checking with you 1st.

At the end of our testing month, Health and Fitness Testing NZ will provide Weta Digital with a company summary report of results e.g. the % of people that had good posture, % of people that had optimal blood pressure, % of people who had diabetes etc. We will NOT be sharing your individual results with them (e.g. we won't be sharing your name or email address with Weta). The purpose of this summary data is to help Weta Digital make more informed decisions regarding future health and wellness initiatives at Weta Digital. By booking your health test/body scan you are confirming you have read and agree to our privacy policy and the sharing of summary results (not attached to your name) with Weta Digital.

Payment

You already paid for your health test online when you booked your appointment. Weta have negotiated an awesome deal for their crew members and Weta have also made a financial contribution towards your health test. 

Cancellations

If you cancel (or no show) within 48 hours of your test, you will not receive a refund.

All cancellations/appointment changes prior to 48 hours pre-test, please email admin@healthandfitnesstesting.nz.

Blood Pressure

We will need access to your entire left arm (skin) to take your blood pressure. Please dress appropriately.

Fit3D Body Scan

Time taken: 15 minutes  

Fit3d Body Scans are also included in the Advanced Test Pack.

You will get the most accurate/best result if you keep your pre-testing and testing regime the same each time you have a Fit3d Body Scan. For example,

  • You will be left in a private room by yourself for the Fit3d Body Scan
  • Wear minimal, TIGHT clothing (e.g. Men = tight, small undies or speedos, women = tight/small undies or G-string and fitting bra is strongly recommended). Scanning in anything loose is a waste of time!
  • Do not perform vigorous exercise in the 2 hours prior to your test. You can exercise after if you like
  • Those with long hair MUST tie their hair in a bun
  • Do not have a large meal or drink large amounts within 2 hours of your test
  • The Fit3d Scanner will take over 1200 photos of you. For accuracy, it is essential you stand still during your body scan. If you do not stand still, a posture report will not be generated. 

Results

The majority of your Fit3D results, including a private online 3D image of yourself, will be available within 5 minutes of your scan (very dependent on internet speed). Your posture report will usually take 15 minutes to process however it can take up to 48 hours. For various reasons, 1/100 scans fail. If this occurs, for any reason, we will re-scan you at no additional charge. 

Username and Password

Please write down or save both your Fit3d and Booking system username and password (Yes, these are 2 different things). When re-scanning, you will NOT be able to scan if you don't know your username and password. 

Advanced Test Pack

Time taken: 30 minutes

If you chose to have an Advanced Test Pack, you will receive the following: Fit3d Body Scan, Finger-Prick Diabetes Test, Finger-Prick Cholesterol Test, Blood Pressure, 1-Page Results Summary Sheet, Discussion with a health coach.

Aside from your posture report (see notes above), all other results will be given to you on the day of your test. 

Anxiety

It is normal to feel anxious before your test. Fit3d Scans alone are performed every 30 seconds in over 60 countries world-wide. If this is your first test, keep in mind, it’s just a baseline :)
